feat(hypr): pin dual-Dell EXTERNAL monitors by desc instead of $EXTERNAL_*

Vertical and double-vertical setups now use stable Dell SE2725H desc
identifiers (BML2X14 left, 34R2X14 right) rather than the env vars, so
monitor attach is robust to dock/hotplug enumeration order.
